Student Care & Advocacy Referral Form

Any member of the University of Dayton community may use this form to share concerns regarding the health, safety, and/or wellbeing of a student with the CARE Team and the Student Care & Advocacy Team in the Dean of Students Office. While this referral may be anonymous, it may limit our ability to respond. Providing your name and contact information allows Student Care & Advocacy to contact you and learn more about your concerns.

This form should not be used to report emergencies. For emergencies that require immediate attention, please call University of Dayton Public Safety at 937-229-2121. Please then submit this form as a means of notifying Student Care & Advocacy of the situation.
